Before we can start work on a project we need to have a written test request from you. It is preferred that you send the request sheet in with the samples but you can also fax it to us at (650) 213-8437. You have several options for requesting tests:

• If your company has a request sheet you can use it. At a minimum it should provide:
  • Your company name, address and phone number
  • The project name and/or number and the project manager's name
  • A list of all samples sent to us and tests to be performed on each sample
• You can use our request sheet.
• You can request tests online in our online reporting system. Simply login and from the homepage click on the button entitled "Request New Tests" in the upper left-hand corner.
